Client Consultation
A comprehensive telephone interview with the employer's HR professional such as the HR Director or Benefits Manager is conducted in order to obtain the answers a list of important of questions developed through the Wrap-TightSM system. This consultation is held in order to qualify the employer's requirements for the preparation of the related ERISA documentation.
Wrap-TightSM Plan Document
The Wrap-TightSM Plan Document is prepared as required by the Department of Labor (DOL) and explains the operating features of the Plan is prepared.
Wrap-TightSM Summary Plan Description (SPD)
The Wrap-TightSM Summary Plan Description, better known as the SPD, is prepared as required by DOL and explains the rights of the Plan Participants and certain features of the Plan including information required by the DOL.
Board of Directors' Resolution
This document is prepared and provided to the legal entity that is adopting the Plan and gives authority to the entity's representative to execute it.
Administrative Procedures for Qualified Medical Child Support Orders (QMCSO)
A procedure guide is provided outlining the employer's responsibility to offer medical plan coverage to certain children of Plan Participants and how a request should be handled by the employer.
Telephone Conference
To the extent requested by the employer, an interim and post document completion consultation phone conferences will be held to review the Plan Document, SPD and any operational issues.
ERISA Pros Binder
A three-ring binder is prepared and provided to the employer including table of contents and labeled and organized printed documents.
Electronic Distribution Package
A Customized Notice, a Statement, Consent Documents and Checklist for Paper and Electronic Distribution of ERISA Documents is prepared.
Form 5500
A Form 5500 is prepared and filed on the behalf of the employer by reconciling all corresponding schedules and electronic filing through the DOL's EFAST2 system
